Letters to Congressmen

Show your support of Hospice, friendly and compassionate end of life care by writing to your congressman. Below are links to letters you can use to write to our elected officials in support of the Hospice Reimbursement Rate. There are letters for each elected official from the "I have used hospice care..." perspective and from the "I have NOT used hospice care...."

Comfort Focused Palliative Care

Family Hospice of Northeast Indiana will offer Comfort Focused Palliative Care. Dr. Roger Lemmen joins the Family Hospice team as Director of Palliative Care. Palliative Care focuses on the reduction of symptons, such as pain, and not on a cure of the disease. Palliative Care may is offered to people with serious or complex illnesses. Comfort and Compassion for People Dealing with Terminal Illness

When experiencing changes in life, Family Hospice of Northeast Indiana wants to be involved in making these changes easier. If you are questioning whether you or your loved one may be appropriate for our services, ask yourself these questions:

  1. Have you or your loved one had multiple hospitalizations or emergency room visits in the last 6 months?
  2. Do you or your loved one utilize medications to manage pain?
  3. Have you or your loved one had multiple falls within the last 6 months?
  4. Are you or your loved one needing assistance with activities of daily living, such as bathing, dressing, walking, getting out of bed or eating?
  5. Has your physician determined you or your loved one to have a life limiting condition?
  6. Are you or your loved one experiencing weight loss?
  7. Do you or your loved one have one of the common diagnoses covered underneath our program?
    • Heart Disease
    • Lung Disease
    • Amyotrophic Lateral Sclerosis (ALS)
    • Liver Disease
    • Alzheimers or Dementia
    • Renal Disease
    • Neurological
    • Stroke/CVA
    • Cancer

If you have answered yes to any of these questions, you or your loved one may be appropriate for our services. Please call us today at 800-355-2817 for your complimentary, confidential in-home visit. You may also fill out a contact form on our contact form and a representative will contact you. Even if you have not answered yes to any of these questions, allow us to provide a complimentary visit with you and your loved one to determine if our services can benefit you or your loved one during this time. During this visit, we can answer any questions or concerns you may have regarding our program.
