What is Hospice Care?
A team of professionals - physicians, nurses, hospice aides, social services, chaplains and volunteers - who develop a plan of care focused on the patient's and family's needs and wishes. Our team makes regular visits to assess the changing needs and to offer support to caregivers.
Our team consults with the patient's attending physician to determine the best way to meet the needs of the patient and family. We provide expertise in pain control and symptom management to keep patients comfortable. We have staff available 24 hours a day, 7 days a week.
