Family Hospice and Palliative Care provides quality compassionate care not only to you, but to your entire family. To reach this expectation, we have several professional teams who are motivated to provide for all aspects of your care. Whether your concerns are medical, emotional or spiritual, our team is there for you, ensuring your needs are being met.

Nursing Team
Once you have made the choice to receive our services, a nurse case manager will be assigned to you to evaluate your needs and develop an individualized care plan based on these needs. Along with the Medical Director, the nurse case manager coordinates your care and assists in symptom and pain management. They will also educate the family and/or caregiver by instructing them on how to care for their loved one while caring for themselves. Whether it is the weekend hours or the middle of the night, this team is on-call to meet every aspect of your care needs, so why not call today and allow us to begin this journey together.

Supportive Care Team
The Supportive Care team offers emotional, psychosocial, spiritual and grief support. This team can coordinate community resources, assist with financial arrangements and aid you in preparing your Advance Directives, if needed. When facing end-of-life issues, each of these team members can help comfort and ease the emotional difficulties. Our journey continues with you after your loss by providing you with bereavement services for up to 13 months and will continue longer if needed.

Volunteers
Generously giving of their time and energy, our trained volunteers can assist you and your family in a variety of ways. Some of these ways include providing companionship, assisting with errands, giving caregivers a break or simply just sitting with you to read, listen and offer that support in times of uncertainty. Hospice Aides
The Hospice Aides play a vital role in our team, enabling our patients to retain their dignity while assisting them with personal care. These devoted team members assist with household duties to allow the patient and family to continue to enjoy the comforts of home, participate in activities with friends and families, and enjoy walks. While providing support and companionship, one of the biggest compliments our hospice aides receive from our patients and families is that they have become a member of the patients' family throughout this journey.
