Our unique history still molds the mission and values of today that drive Family Hospice & Palliative Care (FHPC) to enrich the lives of our community members through compassionate care.

In February 1996, Family Hospice of Northeast Indiana originated as a collaborative commitment of local hospitals from the following counties: Adams, Blackford, Jay, Randolph and Wells.

Family Hospice started as a satellite office of the St. Vincent-Community Hospice Program of Indianapolis. With the passage of the Medicare Balanced Budget Act in 1997, St. Vincent Hospice was determined to be too far to serve Adams County.

Three area hospitals, Adams Memorial Hospital, Caylor-Nickel Medical Center and Wells Community Hospital, joined forces to sponsor and govern Family Hospice as a non-profit corporation in November 1997.

As our needs and goals have evolved, so has the governorship of Family Hospice, which is now maintained by Adams Memorial Hospital, Caylor-Nickel Foundation and Wells Community Health Services Foundation. Today, we serve individuals in the following counties: Adams, Allen, Blackford, Delaware, Grant, Howard, Huntington, Jay, Madison, Miami, Tipton, Wabash and Wells.
